	Complete the course registration form. Two methods are available: 1. If you are a Vision West member, you can enter your Vision West account number and we will bill the fees directly to your account. 2. If you are not a Vision West member, you must enter valid credit card information and we will charge your credit card. Vision West Members: Click here to view and submit exam registration form.
